Emergency intake for services with Laurel Hill Center can be scheduled through behavioral health provider referral.
If you are experiencing an emergent or urgent issue, please call 541-485-6340 and ask to speak to the office based Recovery Specialist.
Community behavioral health providers and integrated clinics may complete and submit the Provider Referral Form securely using the Fill & Submit option or use the Print version to complete and fax it to us at 541-984-3124.
