ACT Medical Coordinator - BSN
Closing date: 2010-08-24 Supervisor: ACT Team Leader
Hours: Contract 15-20 hrs/wk to start - after 6 mos. possibly F/T salary + benefits
Description: The Medical Coordinator provides basic health assessments and education, as well as collaborates with community health care providers regarding the monitoring of physical health conditions, medication side effects and medical equipment or services needed to assist individuals with severe and persistent mental illness symptoms live in the community. In addition the medical coordinator works with the team and participants in developing, implementing and monitoring wellness plans that focus on improving the physical wellness of individual’s receiving services within the ACT Team.
Responsibilities: • Attend daily team meetings and participate in the development and implementation of planful interventions designed to address barriers to community integration
• Provide medication administration, monitoring and support to participants
• Provide basic health screenings
• Coordinate with PCP, medical specialists, hospitals and rehabilitation programs regarding medical recommendations
• Consult with the ACT team regarding the medical recommendations of community health agencies to ascertain the capacity of the team and the participants to meet the discharge recommendations
• Coordinate with community providers to obtain needed medical supports such as care-givers, medical supply ordering and maintenance
• Provide in-home skills training to participants to monitor and effectively manage medical conditions such as diabetes, COPD and Hepatitis
• Provide support to the psychiatric nurse practitioner in the administration of injectable medications and cholesterol testing
• Provide individual and group skills training regarding nutrition, medications, and incorporating physical health as a component of recovery
• Participate in trainings and meetings
• Comply with Laurel Hill Center policies and procedures in addition to Medicaid, ISSR and HIPPA standards

Requirements: Registered Nurse from an accredited college or program with the ability to obtain a credentialing of Qualified Mental Health Associate within 3 days of employment. The position requires experience with individuals who have been diagnosed with a psychiatric illness or coursework in psychology. Current driver’s license and a good driving record; flexibility, strong verbal and written communication skills, problem solving and negotiation skills
Outreach Counselor
Closing date: 2010-08-27 Supervisor: Team Leader
Hours: Full-time
Description: Outreach counselors provide skills training, case management,
crisis intervention, and support to participants who reside in the community. Laurel Hill Center mission and values stipulate that all services provided are based on participant strengths and choices and follow evidenced based practices of psychosocial rehabilitation.
Responsibilities: 1) Conduct all phases of psychosocial rehabilitation and outreach (engagement, assessing strengths, goal planning, safety planning, evaluating and summarizing progress)
2) Work to uphold Laurel Hill Center’s established values and beliefs, utilizing evidenced based practices when delivering services to participants
3) Actively support participant self-reliance and use of natural supports
4) Provide skills training for medication management, financial management, and for all activities of daily living
5) Provide case management and crisis intervention services
6) Provide supported housing assistance
7) Collaborate with other staff members, community agencies/supports, and family members about participants mutually served
8) Complete required documentation in a timely fashion including the strengths assessment; personal treatment plan; safety plan; domain changes; and progress notes
9) Exhibit competencies gained through evidenced based practice training modules
Requirements: • BA/BS in mental health, social services, or related field, plus one year experience relating to or assisting individuals with mental illness
• Must have current and valid Oregon driver’s license in accordance with Laurel Hill Center vehicular policy and procedure
• Documentation of successful completion of a background and fingerprint check as specified by Oregon law as well as a urinalysis drug screen
• Must obtain credentials from LaneCare as QMHA within the first three months of employment at Laurel Hill Center
• Must be able to manage the physical demands that include getting in and out of vehicles, climbing stairs, and lifting/carrying 25 pounds or under
